Bad brake pad wear

What would cause the brake pads to wear this way? These are the pads that came from factory. Referring to the missing material on the corners opposite of the pin inserts.

If you're referring to the "missing" material in the corners, I think that is how they are manufactured.

To me, it looks like a radial J-scoop chamfer used to reduce vibration and noise:

Mine are that way. I asked at the dealership and had one of the techs take a wheel off and that was how it was too. Helps to remove squeal and vibration IIRC
